As requested by intuit, all applications that intend to access Quickbooks online production data are now needed to fill in and submit a questionnaire. Please see below on how to answer the app assessment questionnaire.
To view or fill in the App Assessment Questionnaire, navigate to Production Settings >> App assessment questionnaire after selecting the app for which you need production access from the Intuit developer portal.
The submission status and the results will also be displayed on the same page.
The number of questions you will be required to answer varies depending on the functionality of your application. Please plan on spending 20 - 30 minutes to complete the App assessment questionnaire.
The App assessment questionnaire will be divided into the following sections. The sections that will be visible to you will depend on the answers you provide and the regulated industries that use your app.

You will be able to answer these questions based on your company background.
Insurance
If the app is in the Insurance industry and is offering insurance products or services, either directly or indirectly, then these questions will have to be answered by you.
If this app does not include any functionality related to Insurance, please review and update your regulated industries settings under the Production settings of the app.
App Information
Please see the answers that we suggest for the app information section of the app assessment questionnaire.
Select the option "You were asked to create this app in order to get credentials/keys to be used on another platform that integrates with QuickBooks" for the question " Which of the following is true about your app (at least one option must be checked)"
Please use the below link when you are asked to enter a link to the instructions on which you were asked to create the Quickbooks Online application,
When you have completed the questionnaire, you can click on the "Submit" button to submit the app assessment questionnaire. You do also have the option to save your answers and review them at a later date before submitting them.
